How to Apply

Listed below are links to all required forms and relevant information. In order to apply, first fill out the Application Form and mail it in along with your registration fee to secure a seat in a course(s). Once you reserved your space there are additional forms (listed below) required to finalize your application.

The Application Form is available 3 ways:

1. Online Application. Click on link below to fill out and submit online. If you complete an online form you will receive a 'cc' to your email address. Verify this information, then edit as needed; print, sign and mail it in along with the registration fee to reserve your seat.

2. PDF Application Form. Click on link below for the PDF Application Form. Print, complete, sign the PDF application and then mail it in with the registration fee.

3. Application Info Pack By Mail. If you request an Info Pack you will receive a printed application that you may complete by hand. Complete, sign and mail it in with the registration fee. Upon request we can also email you a Word version. Request Info Pack at info@lasuerte.org
